Required documents for Chinese visa:
▪ Original passport. (Passport must be valid for at least 6 months and have at least 2 blank visa pages).
A copy of the personal information page of the passport.

A copy of the applicant's most recent Chinese visa or Chinese residence permit.
A copy of the applicant's address in the U.S. (e.g., driver's license, utility bill, bank statement, etc.).
▪ If the applicant is a citizen of a third country in the U.S., a copy of proof of legal stay or residence in the U.S. (e.g., green card, visa, I20, I94 form, etc.).
▪ If the applicant had Chinese citizenship and this application is the first Chinese visa application after joining a foreign country, please provide the original of the original Chinese passport and two copies of the personal information page and one copy of the naturalization paper.
▪ If the applicant's name has been changed and this is the first application for a visa to China after the name change, a copy of the name change document must be submitted.
