A copy of the applicant's proof of address in the U.S. (e.g., driver's license, utility bill, bank statement, etc.). A copy of the applicant's Where You Stay Form (please print below and sign by hand). 7. If the applicant is a citizen of a third country in the U.S., please provide a copy of the proof of lawful stay and residence in the U.S. (e.g., a green card, a visa, an I20, an I94, etc.). 8. If the applicant had Chinese citizenship before, and this application is the first application for a Chinese visa after joining a foreign country, please provide the original Original Chinese passport with two copies of personal information page and one copy of naturalization paper. 9.
